This is what you need Ken. Just omit the tone control.
I recommend using a 3 way switch when combining a piezo and mag.

Use his search for wiring diagrams. There are several topics listed on this subject so you'll have to navigate a little to find the diagram you need.
I would suggest wiring & soldering on a template or thin cardboard, then plug it in for a test prior to installation, this gives you room to correct any error before cramming it into the tight pockets in the guitar body.

im new and a puppy in the big dog park. It seems that folks show glorious photos of outside finish, but none of the mechanics in the guts. I have a bridge with rod piazo and 1/4" plug. Now what? I see pre-amps at Gitty. and other bits, is a pre-amp needed? Better question is "How does this stuff work" and what is a good book for CBG wiring? Im making Uke's and 3 strings for starters, some of my boxes are too thin for acoustic, so this is all new.
On a Gibson I have, the strings are about 3/16ths of an inch (sorry, not good with metrics yet) above the humbuckers. On an Ibanez I have with single-coils the string are closer to an 8th of an inch. I don't think I would go higher than around 3/16ths to a quarter inch. Any higher and I think you'd be sacrificing signal strength/volume.
jeff Cadzow I am not sure if you are making this so the pickup height is adjustable. If so, then you can Google, "String height above pickups". You will find all sorts of good info there within the first few sites (including a YouTube video with some tips and tricks.
