For first build I am using extended 1/12" for tailpiece with three holes and grommets. Was thinking about angling the holes at a 45' row. I assume placement of strings at end piece doesn't matter even though all will have if different lengths and tuning determines tone?

yes angle them how you want, it is the position on the bridge that is important, have the bridge angled towards the high string side and adjust the bridge back until  you find the correct intonation off the 12th fret :-) 

What about some pics I have seen where 1&3 holes are in line and #2 is ahead or back? Would this require a different bridge angle?

The position of the strings at the tail doesn't matter. It's where they cross the bridge, and how far the bridge is from the nut, that matters. You can attach the strings at the tail all in a line, or angled one way or t'other, or with two back and one up, or vice versa. Do 'em how ever looks good to you.

Oh, and BTW, that bridge IS angled, if you look closely. It appears to be angled the wrong way, for a right handed git with the thickest string to the left, but sometimes intonation works that way.

Has absolutely nothing to do with where the strings attach to the tail.

There is only 1 real important consideration in locating the string anchor/termination/ferrels/etc in the tailpiece.

If the pull from the strings is not balanced, the bridge will move sideways and the strings won't line up neatly down the fingerboard. 

keeping that in mind and you can use any pattern your eye finds pleasing... inline, diagonal, forward V, inverted V, Lazy W, reclining L, trapeze, flyingmonkeyswithabluecape, etc.
