i had this years ago. it did a very good job.

its an avery product so the templates should still be available. 

you can buy printable CD's and DVD's my canon inkjet printer came with an adaptor so i could print right onto the disk. you could download the program from canon. other brands had the same sort of attachment.

casio has a cd printer that prints right on the cd itself. It works pretty well for the most part, or you can get the peel n stick avery labels.
